Information you choose to share

The contact form asks for your name, who the request is for, your preferred contact method, email address or phone number, state, coverage interest, best time to reach you, and permission to contact you. Please do not submit Social Security numbers, Medicare numbers, payment details, or medical information.

How a contact request is used

Whidbee Insurance Group uses your request to respond about the coverage topic and contact method you select. The form is a request for a conversation, not an insurance application or enrollment. Giving contact permission is not a condition of purchasing insurance. You may call or book an appointment without submitting the form.

Form processing and website hosting

The contact form is processed by Web3Forms. When you submit a request, Web3Forms receives the information you entered and delivers it by email to Whidbee Insurance Group. Authorized administrators of this website may also receive requests to manage the service. Web3Forms may process technical information, such as your IP address and browser information, to deliver requests and prevent spam, under its own Privacy Policy.

Website hosting and delivery providers may process technical information such as IP addresses and request logs to serve and protect the website. The website has no advertising pixels. It may use Cloudflare Web Analytics, a cookie-free service that counts visits and page views without tracking you across other websites.

Other websites and contact methods

Links to PlanEnroll, Cal.com, Calendly, Facebook, Medicare.gov, and other websites take you to separate services. Information you provide there is governed by their privacy practices. Email and telephone links open your email or calling application.

Your choices

To ask about information you submitted, request a correction or deletion, or withdraw permission for future contact, email [email protected] or call 252-254-1243. Please do not include sensitive identifiers in your request. Information may need to be retained to meet applicable recordkeeping requirements.
